WAIVER FORM

Please read and sign below:

This Waiver must be accepted and signed in order to join in on rides or any other activity with the group.

In submitting this application and/or by taking part in any group related activity, I hereby make known that I will hold blameless and harmless in the case of accident, injury, or damage of any kind, the Muddy Bums Mountain Bike Group Of Northumberland, its members, associates and volunteers (together herein known as “the group”). I recognize that mountain biking is potentially dangerous, and I represent that I am a competent cyclist with safe equipment. I understand that I participate in the group’s activities at my own risk. I further recognize that my safety whilst engaged on group activities is my personal responsibility and not the responsibility of the group or other members of the group.

I acknowledge and agree that any guidance as to route selection, riding techniques, weather conditions, equipment maintenance & repair or any other feature, activity, assistance or advice given and received whilst engaged on group activities is provided without responsibility on the part of the group members for the accuracy, suitability, safety or reliability of such and it is entirely my responsibility to either accept and / or act upon the guidance, assistance or advice or not. I hereby make known that I will accept full and personal responsibility for my own decisions on whether to take and / or act upon such guidance, assistance or advice or not.

I agree to hold the group and its members harmless and indemnify the group and its members for all costs, judgments and awards that may be claimed including the cost to defend such claims brought by you or any other person or organization on your behalf or that of others.

I confirm that I am over 18 years of age and represent that I am sufficiently fit to take part in group activities.
